Work with the hospice care team to provide excellent health care service to patients. Communicate with patient’s family members to provide a plan of care. Assist patients with bedpan changes, diapering and emptying drainage bags. Monitor and record patient stability by checking vital signs and weight. Assist in ambulation and exercise as instructed by the hospice care nurse or therapist. Provide personal care hygiene to patients that includes baths, back rubs, and oral hygiene. Report any changes in patient condition to hospice nurse.
