4. Driver’s License or Picture ID

5. Social Security Card

6. Employment Authorization (e.g. Alien Card, Voters Registrations Card, Passport or Work Permit)

7. Current HIV CEU

8. Domestic Violence CEU

9. For all C.N.A and HHA an additional 12 Hours of In-Services is needed each year

11. e.g. OSHA, Residents Rights etc. (NO Assisting with Medication) 12. Current Physical within the last six (6) months signed by a doctor which states you are free of any kind of communicable diseases including Tuberculosis. 13. Covid-19 Vaccination Card or Exemption form

14. Copy of car insurance

15. Auto Policy

16. Professional Liability Coverage for C.N.A/LPN/RN can also be completed online at www.cmfgroup.com

17. Level 2 background check.
