I have been a registered physical therapist from 1968. My license expired in 2009 and I did not renew it as I was working in our family business.
As a registered physical therapist, I have worked in Hospitals in St. Paul, Minnesota, Walnut Creek, California, and West Palm Beach, Florida. I’ve worked with handicap children with Easter seal in the school system in Miami, Florida and as a visiting therapist in Beaufort, South California and most recently worked with brain injured people that no Nu Medico in West Palm Beach. I allowed my license to expire as I helped our family business develop products for use in medical areas in Asia and the Bahamas.
I would like to renew my license as I would like to work in a hospital setting or a nursing retirement facility or with young children, I do not know what it would take to get my physical therapy license renewed or possibly a PT Assistant licence would be fine. I’m submitting this application and my experience and credentials and hope to hear back from you as to what I need to do in order to get registered as either a PT assistant or a physical therapy registration thank you so much for your attention
