Where applicable, in accordance with the Employer's Background Screening Program and in compliance with applicable laws regarding pre-employment screening, I provide my consent to the Employer to use my name and email address for the purpose of contacting me to initiate this process. I understand that neither the Employer nor the vendor will commence background screening without my further express consent, which will be sought should my candidacy for employment progress. I understand that any employment offer made to me is contingent on the satisfactory results of the pre-employment background screening. I understand that should an employment offer be extended to me and accepted that I will fully adhere to the policies, rules and regulations of employment of the Employer. However, I further understand that neither the policies, rules, regulations of employment or anything said during the interview process shall be deemed to constitute the terms of an implied employment contract. I understand that any employment offered is for an indefinite duration and at-will and that either I or the Employer may terminate my employment at any time with or without notice or cause. I also understand that no manager or other representative of the Company has any authority to change my at-will employment status except the President, and then only by a written employment agreement expressly for that purpose. I understand that the Employer has the right at all times in its sole discretion to change wages, benefits and working conditions, with the exception of the Company’s at-will employment policy, as described above. I further understand that as a condition of employment or continued employment, I will be required to provide proof of work authorization and identity as required by law. I understand that any employment offer made to me may be contingent on my taking, and passing, a pre-employment drug-screening test if applicable for the position. I will not be hired if I do not pass. If I have started work before the Company receives positive test results from my pre- employment test, my employment will be immediately terminated. I further understand that the Employer reserves the right to require current employees to submit to a drug test if: (a) reasonable suspicion exists that an employee is using or is under the influence of illegal drugs or alcohol while on duty, or (b) an employee in a safety sensitive position is involved in a workplace accident that requires immediate medical attention. I understand that applicants for warehouse positions are also required to pass a pre-employment physical examination and that a n offer of employment is contingent upon my ability to perform the essential functions of the particular warehouse job. Finally, I understand and agree that if I am hired, any dispute I may have relating to my employment shall be subject to final and binding arbitration in accordance with the Company’s Arbitration Agreement.
