OBJECTIVE STATEMENT
Osuagwu Beatrice Chizoba is a dynamic person with over 3 years’ progressive experience in HIV/TB Programming, and also a Media expert. My mission in life is to work with institutions that are involved in improving the health and lives of the most vulnerable and disadvantaged population through capacity building, operational researches and services provision in the area of Care and Support, OVC, Nutrition Programing.
PROFESSIONAL EXPERIENCES/EMPLOYMENT HISTORY
A.Ccommunity Case Manager CCFN 4GATES project
Centre for Family Health Initiative
{October 2018-September 2022}
Mobilized, sensitized, counseled and tested 65 adolescents and children enrolled into 4GATES project for HIV and referred those positive for enrolment in care in comprehensive health facilities.
Provided community based adherence counseling support through home visits to 105 HIV positive beneficiaries on ART (children and caregivers).
Provide Emergency support to the PLHIV to obtain Medical Traetment,
Provid Logistic support monthly for drug pickup and for sample collection for Viral Load test.
Facilitate Home Garden Training for the Cargivers enrolled in 4GATES project.
Facilitate food Demonstration the Caregivers in their communities on how to cook locally available food within their environment and also Nutrition to their body
Update/ Track Viral Load and next drug Refill of the enrolled beneficiaries,
Coordinated advocacy meetings, community dialogue sessions and community mobilization activities among traditional structures, government agencies and institutions towards program sustainability and ownership
Coordinated high standard adolescents’ friendly HIV programs that motivate adolescents to seek care.
Provide Nutrition Support to the Malnurished OVC .
Incharge of weekly and monthly report of the activities carried out.
Ensure that all PLHIV Consent forms are signed from their various facility where they take ART.
