Pre-admission Screening Resident Review for patients that are admitted to a nursing facility from an acute hospital, psychiatric hospital or community setting with a diagnosis of Mental Illness or Intellectual Disability. If the client is positive for PASRR Specialized Service for Mental Illness or Intellectual Disability, then a service coordinator will be provided by Andrews Center for services to address the client needs. These services are provided while living in nursing home.

Case Manager: My job description was to manager twenty client caseload. I assisted and advocated for client services within the community. Those services consisted of client rights services for medical needs, medical supplies, attend Individual Team Meeting with local school official (teacher, principal, and counselor) local Mental Health & Intellectual Disability Agency, and legal services (if needed).
Management: I manage up to six group homes and thirty staff personnel. My duties were to make sure that the staffs were providing appropriate services to all clients. I was responsible to make sure that each staff receive that appropriate training and skill to meet the needs of that client. I was responsible to make sure that each staff understand the requirement of company policy and procedures. I was responsible for hiring process & procedure and follow termination process and procedure.
