Inland Regional Center serves over 33,000 consumers with developmental disabilities. Under the direct supervision of the Program manager. I am responsible for coordinating service and supports available to consumers as outlined in the Lanterman Developmental Disabilities Service Act. I carry a caseload of 63 and case coverage as needed. This means that I maintain ongoing contact with consumers and their families to ensure needs are met.
At Inland Regional Center I am responsible for assuming daily Consumer Services Coordinator responsibilities:
Maintain contact with consumers and their families as required.
Evaluate consumer progress and plan as identified in the Individual Program Plan (IPP) on an ongoing basis.
Pursuant to the State Department of Developmental Services (DDS) provide advocacy services to consumer and their family. Give high priority to attending all relevant meetings (i.e. IPP, IEP) and prepare paperwork and notate the IPP as necessary.
Seek out and effectively utilize generic resources on behalf of consumer. Provide information and referral services.
Coordinate consumer assessment including nursing, medical, psychological/developmental, education or vocational and others, and follow-up as needed.
As assigned, serve Medicaid Waiver cases completing required documentation in a timely manner.
