• Receive report for assigned unit and count the narcotics at shift start correcting any discrepancies before accepting the unit.

• Evaluate residents in Long Term Care, Rehab, Post-OP, Psych, New Residents during adjusting, and hospice patients on comfort measures for any changes or abnormal behaviors/responsiveness. Call MD to give evaluation and to receive further orders. Call the Responsible Party of all changes.

• Supervise nurses' aides or assistants.

• Work as part of a healthcare TEAM to assess patient needs, plan and modify care, and implement interventions.

• Evaluate nursing intervention outcomes, conferring with other healthcare team members as necessary.

• Admission Paperwork *Greet new resident *Inform MD that resident has arrived *Show Patient to their room *Educate on safety measures and call bell/light *Assessment's *Answer questions (Family/Resident)

• Medicare and Acute charting

• Administer prescribed and As Needed (PRN) medications within time frame allocated, noting times and amounts on patients' MAR's

• MAR change over at the end of each month. Making sure all new orders are transcribed and discontinued or changed orders are updated for the new month. Clarifying orders if the chart doesn't have an updated order but the MAR has a change or discontinued order. Making sure all Allergies, Doctor's names and Patient names match.

• Observe patients, documenting and reporting changes in patients' conditions, such as adverse reactions to medication or treatment, acute episodes, and taking any necessary action within my scope of practice to resolve matter within company protocol. Make MD and RP aware of change

• React without hesitation in emergency situations, such as, Physical, Emotional, or Verbal Abuse on residents and staff, falls, Full Codes. Know where to find code status on patient, activate CPR and 911. Call MD, write order to send out to Emergency Room. Notify family and On-Call Nurse. Call hospital to give report and document events and times. Call 2-4 hrs later for follow-up.

• Assigned Body Audits/Skin Assessments

• Measure and record patients' vital signs, such as height, weight, temperature, blood pressure, pulse, and respiration.

• Provide basic patient care or treatments, such as dressing wounds, treating bedsores, suture removal, giving enemas or obtaining specimens through catheterizations.

• Assist patients with bathing, dressing, maintaining personal hygiene, repositioning in bed, standing, walking, or assisting with Hoyer Lift transfers X2 Assist.

• Assemble and use equipment, such as catheters, tracheotomy tubes, or oxygen suppliers, Ambu bag, spirometer, nebulizers and CPAP's.

• Collect samples, such as blood, urine, or sputum from patients, and perform routine laboratory tests on samples.

• Receive and check over labs faxed, notify MD of any critical labs that couldn't wait until the next day. Usually PT/INR results

• If patient has a contagious illness, maintain contact precaution.

• Apply compresses, ice packs, or warm rags.

• Get all paperwork copied and ready for all appointments scheduled as late as a 10:30 A.M. P/U time

• Check all medications, OTC house stock, medication carts, and refrigerated medications such as insulin and eye gets for expiration dates. If an order is changed or discontinued, remove the old medication card from the cart. Check medication over flow. Fax pharmacy to refill medications and to discontinue medications. Fax new admission MAR to pharmacy to receive medications for new residents, fax communication slip to the pharmacy when using back up kit. Accept medications from pharmacy on delivery and count new narcotics. Sign off pharmacy transaction sheet. Put medications away. Licensed Practical Nurse
