Green Oaks Psychiatric Hospital: 7808 Clodus Fields Dr., Dallas, TX 75251 I was employed as a Treatment Manager at this facility from August 2006 until March 2008. I worked in numerous departments, such as psychiatric triage, inpatient management of treatment, assessments and discharge planning. My experience there was inpatient acute care as well as emergency room settings, providing high quality discharge planning to maximize the benefits of their hospital stay. Value Options

I was employed as a Clinical Care Manager here from October 2, 2000 until May 5, 2006. I began as a temporary Clinical Care Manager through Social Work PRN in June, 2000 and accepted the opportunity to join the Northstar Team as a permanent employee. I conducted concurrent reviews and admission intakes for various levels of care. My primary responsibility began as care manager of Nexus Chemical Dependency Treatment Center for women and their children.

I worked on a contract basis at this skilled nursing facility from January 1, 2004 until January 1, 2006. The majority of my clients were elderly but all were suffering and recovering from various illnesses and disabilities. My role at Ashley Court was primarily performing assessments of new patients but also involved building therapeutic alliances with the residents in order to make recommendations for improvement of care and to maximize outcomes.

Parkland Health and Hospital System

I took part time employment on weekends and holidays as a Social Worker from September 2001 through February 28, 2006. I provided counseling, crisis intervention and discharge planning in all populations including women and children, pregnant and post partum females, rape and domestic violence victims, geriatrics, critical care, burn patients, the homeless and ER response. I worked with the patients and their families to optimize their experience at Parkland and to get support them in getting through a tough time in their lives.

James L. West Presbyterian Special Care Center

This was my first official job as a licensed Social Worker from October 1996 until June 2000. I provided support for families of dementia patients and their staff. I assisted in quality assurance of care by monitoring for abuse issues, special needs, care planning and coordinating care with all professionals involved. I was also responsible for assuring that all physical, emotional, social and spiritual needs were met for each patient in this 120- bed facility. I also provided in-service trainings to the staff discussing the proper ways to address the special emotional and mental needs of residents and their families to improve the quality of their lives.

Alliance for Children

I worked as the Center Coordinator from July 1996 until October 1996. This position was to assure the smooth operations of daily care provided to children who had suffered abuse to enable them to tell their story in a child friendly environment and facilitate the interviews of perpetrators in a multidisciplinary agency.
