Wellbe Senior Medical, Independence, Ohio: Full time Nurse Practitioner in Primary Care. Collaborates with specialists in caring for the geriatric population. Home based assessments as well as in assisted living facilities. Assess, diagnose, prescribe if needed. One night of call rotation about once per month. Prescribing to fill medication gaps. Pure Healthcare Palliative Ohio’s Hospice (Life Care) Wooster: (Full Time nurse Practitioner 06/08/2022-present): Assess, diagnoses and intervene with symptom management of chronic and/or terminal illness. Seeing patients in their homes in seven counties surrounding Wooster. Collaborates with primary care providers, oncologist, nephrologist and all specialists managing patient care. Assessing and educating patients and families on code status and defining each one while interviewing patients and families on their end-of-life plan/goals. Educating patient and families and expected outcomes with their disease process while preventing readmissions by monitoring for early interventions of exacerbations of their disease. Assessing patients for hospice appropriateness and providing information and education on the hospice benefit and resources. Refers to appropriate providers including but not limited to social workers, chaplains, volunteers, specialists when appropriate, Works on-call hours one night a week and one weekend per month taking calls from Hospice field nurses managing and prescribing for end-of-life medications. Job duties include but are not limited to assessing members post discharge for any immediate needs and assisting with coordination of care. RN provides education on all chronic and acute conditions and how to prevent readmission to the hospital. RN provides medication education. RN utilizes HCAT and CGX 2.0 to review diagnosis and document each call. Collaborate with interdisciplinary team to assess member needs. Currently manages a roster of 300 Humana members creating care plans and assessing member’s ability to recognize signs and symptoms of their conditions and provide education on interventions.

Registered Nurse Case Manager

Job duties include but are not limited to Oasis documentation for every patient admitted, recertified, or discharged. Allscripts software documentation for all patients seen. RN assessed patients for start of care, resumptions, routine visits, transfers, and discharges. Education of patient medication and compliance, medication requisitions were completed and MD contacted if there are any medication discrepancies. RN placed orders for PT/OT/MSW/Heart care at home/WOCN disciplines and report to disciplines as necessary. 5-6 visit productivity required 5 days a week. Case management of 25-30 patients was the productivity goal. On call one night a week and every third weekend coverage as required. RN traveled to each patient’s home and assessed safety and environmental hazards. Reported to proper agencies when problems are found. Management of therapies within the patient home included but not limited to wound vac dressings, PICC and central line dressing changes weekly and lab draws. IVIG, IV antibiotics and venipuncture procedures. Collection of urine and stool samples when ordered. Foley catheter and management when ordered. Job duties also required collaborating with MDs when there is a change in patient status or when the plan of care is changed. RN educated patients and families on disease processes of all kinds such as cancer, wound care, activity intolerance, diabetes, stroke, congestive heart failure and post- operative care. Interventions and goals were discussed with each visit.

Patient population includes but not limited to strokes, cardiac arrest with arctic sun protocol, diabetic ketoacidosis, narcotic drug overdoses and delirium tremor monitoring. Assists physicians in sterile field protocol to insert arterial and central lines. Zeroing and recording said lines every four hours. Responsible for all collaboration with intensivists and attending physicians in coordinating patient care and reporting any changes in status. Job description entails working closely with family members of the critically ill and caring to their needs as well. Participating in code blue and rapid responses throughout the hospital system.

Neurological ICU Registered Nurse

Completed intensive orientation to Neuro ICU. Hourly neurological assessments with vital sign checks every 20 minutes for acutely ill patients. Patient population includes but not limited to intracerebral hemorrhage, subdural hematomas, subarachnoid hemorrhage, ischemic strokes, brain death donor protocol, brain tumors, myasthenia gravis and baclofen pump failures. Post surgical care of patients who have had evacuations of tumors and thrombotic removals. Initiating and following arctic sun protocol for medically induced comatose patients. Accompanying transport of these critically ill patients to CAT scan, MRI, and operating room. Patient care includes but not limited to external ventricular device (EVD)/intracranial pressure monitoring (ICP), central venous pressure device monitoring, Schwann Gantz and intra-arterial monitoring, blood draws, bathing, and hourly input/output monitoring. Primary nursing care for patients who are on ventilator support through either tracheotomy or oral intubation. Responsible for dressing changes for pressure ulcers, skin tears, and post surgery. Assessing and monitoring patients who have multiple chest tubes. Troubleshooting all monitoring devices regarding all instruments mentioned.
