RN Hospital Case Manager

Currently work full time as a Hospital RN Case Manager. I assist with coordinating inpatient and outpatient care for patients. I also review patients’ charts and meet with families and healthcare providers to establish the most suitable plan of care for each patient. I complete intake assessments with patients and families. I develop individualized discharge plans that account for the physical and financial needs of each patient. I do meet with multiple members of the healthcare team including physicians, administrators, and physical therapist to discuss patient’s progress on a daily basis as well.
