Primary Care Provider duties (0.8 FTE):

Engaged in primary care and walk-in (urgent care) services for patients of American Indian and Alaskan Native descent. Maintained a primary care panel of approximately 600 patients. Served an average of 14 to 15 patients on days dedicated to primary care; and an average of 18 to 22 patients on days dedicated to walk-in clinic (urgent care).

Determined and assessed the health status of patients by reviewing records, taking comprehensive medical histories, performing physical examinations, and recording findings. Planned and oversaw on-going treatment and clinical management and general health maintenance of patients.

Ordered routine laboratory work, screening, and diagnostic tests such as plain film x-rays, EKGs, and spirometry and interpreted results. Placed orders for advanced imaging when appropriate.

Performed in-office procedures such as joint aspiration and joint injections, incision and drainage of abscesses, Nexplanon insertions and removals, pap smears, shave and punch biopsies, suturing and repair of minor skin lacerations, foreign body removal, splinting, fluorescein staining, cryotherapy, and other procedures appropriate for patient’s condition.

Informed, counseled, and instructed individuals and families on their specific health problems, test results, and general health-related matters such as nutrition, birth control, social behavior, growth and development, aging, and substance abuse.

Prescribed medications to include schedules II, III, IV, and V drugs according to proper clinical care and standards of practice.

Performed inductions and maintenance of buprenorphine Medication Assisted Treatment (MAT) in combination with counseling and behavioral therapies for patients with opioid use disorder. Out of the patients I inherited in my first year of employment, I inherited I achieved a 30% decrease in the number of patients on chronic opioid medication.

Conducted annual wellness visits and preventive care services for health promotion and disease prevention.

Performed Department of Transportation (DOT) physicals as a registered NRCME provider. Also performed other employment physicals and sports physicals.

Recognized situations requiring a specialist referral and provided a letter of consultation to that specialist to ensure continuity of care.

Provided emergency medical treatment as a BLS, ACLS, and PALS certified provider when necessary.

Research

Past Research. I did a quality improvement project at Klamath Tribal Health and Family Services for my Doctor of Nursing Practice degree. The project was entitled “Prescription Refill Management Quality Improvement Project Utilizing Standardized Protocols for RNs in a Tribal Clinic”. The aim of the project was to achieve a more streamlined method for prescription refill management by employing evidence-based, RN-guided prescription refill protocols. Implementation of the protocols was to decrease the workload for the providers by allowing RNs to refill prescriptions using predefined algorithms. The project contained measurable objectives and aims with varying methods of evaluation (surveys, pretests and posttests, paired t-tests, questionnaires) to determine if the goals of increased consistency in refill management procedures; and improved patient, provider, and nurse satisfaction with the refill processes are achieved. The date of completion for this project was July 2017. The project was a success, and the nursing staff was able to handle 78% of refill requests using the predefined algorithm, successfully reducing provider workload.

Past Research. As a nurse practitioner student at Delta State University, I completed a research project in 2011 entitled “A Replication of the Breast Education and Early-Detection Project (B.E.E.P.): A Comparative Analysis of Pre- and Post-Breast Education Program Participant Learning.” The premise of the research was centered on breast health education, health promotion and self-care. By the end of the research period, there was a significant increase in breath health post-test scores compared to pre-test scores, and participants reported more self-breast examinations, clinical breast examinations and mammography screenings in the three-month period immediately after the education sessions.
