CS360 Health Care Program Nurse Practitioner
Works closely alongside an interdisciplinary team composed of case manager, social worker, pharmacist, registered nurse and medical director in order to provide closer follow up and medical care to a complex patient population with multiple medical comorbidities coupled with frequent hospital readmissions, urgent care and ER visits, referred primarily by PCPs or specialists. Initial approach is weekly medical follow up visits for about a month and frequency tapers depending on patient’s progress and health care needs to help reduce readmissions. Rest of team will continue reaching out weekly to continually provide patient/ family support and navigate care

Designated as the lead NP for the Enhanced Care Program (ECP) since its 2nd pilot study launch. Successfully expanded its services from a single SNF to a total of 8 SNFs. Work in close collaboration with over 105 private/community physicians towards achieving goal of preventing hospital readmissions of SNF patients via utilization of advanced nursing skills, having an open communication with providers for early detection of any health concerns and providing a calm and comfort environment for patients and families.
Once discharged home, ALF, board & care within 10 miles radius, NP home tuck-in visit is rendered within 24-48 hours to assist with safe transition back home by performing complete physical exam, assess social and home safety, medication reconciliation, connect with case manager and/ or social work if involvement needed and keep an open communication with PCP. Responsible for precepting and orienting new NP hires, clinical educator, case managers.
