Dear Jo-Ann,

It is with great pleasure that Well Care Hospice offers you the position of Hospice Registered Nurse Case Manager, seeing patients in the Triad Hospice Territory. Our service area is a 60-mile radius of the Bermuda Run, NC office and during peak times you may be asked to provide care to patients outside of your assigned territory. You will be reporting directly to your Clinical Manager. As a Hospice RN Case Manager, you will be required to give your availability to your Clinical Manager and Team Assistant monthly. Based on your experience, we are offering you $40 hourly with biweekly pay. Mileage is paid at the agency rate of $0.50 a mile. We will provide a tablet, cell phone, and all supplies needed. You will not be required to do night on call.
