February 2012-April 2012 As a special Nurse Practitioner Consultant, I traveled to 6 separate skilled nursing facilities in the Greater Sacramento Area assessing patients for many common Geriatric syndromes and providing prudent medical management recommendations to both the staff and the primary MDs. I frequently performed complete cognitive assessments on brand new admissions to assess for the presence of very common geriatric syndromes such as dementia, delirium and depression. I also assessed previous Vericare admissions and attempted to manage their conditions with as little medication as possible, always attempting to develop non-pharmacological methods for dealing with their syndromes when possible. When medication was needed, I performed a careful medication review, watching closely for potential medication interactions, attempting to reduce polypharmacy and attempting gradual dose reductions of psychotropic medications when it is possible. I consulted with patients, families, staff members about these syndromes and provided teaching to help all involved better understand the behavior of the patients.

2009 -2010 I identified ideal high-risk patients, both geriatric and adult, and subsequently spent full days providing home centered care with the goal of keeping this group of very fragile, medically complex patients who were most likely to be hospitalized repeatedly as determined by their Risk Stratification Tool used at Health Care Partners. These patients most often had several complex and advanced medical conditions and an often had no transportation to bring them in to the clinic to see a provider. Focused on teaching caring for patients using critical thinking skills for decision making. I additionally provided daily oversight of all unit clinical operations and expert care to patients on the unit with special attention paid to those veterans that required specialized geriatric interventions or who were faced with one of many common geriatric syndromes or who required extra specialized care. Worked certain days entirely “on the floor” as a staff RN to gain insight and to form needs assessments as to where deficiencies were in the clinical care being provided on the unit. Using this information, I provided on the spot education or devised new in-services and or/ competencies for the nursing staff. I answered geriatric consult requests from all areas of the hospital including the medical and surgical units, the rehabilitation floor as well as in the critical care units. I both wrote orders and made recommendations to Internal Medicine Resident Teams on how to more appropriately care for geriatric veterans. Educated resident teams on common geriatric issues, conditions, syndromes, treatments and solutions. Maintained close working relationship with WLA’s VIP Palliative Care Service. Was appointed by DON to Special Task Force on Fall Prevention. Devised new, comprehensive fall prevention program to be implemented hospital wide.
