o Provides patient centred nursing care to patients/residents with a variety of physical, emotional and/or behavioural problems.

o Observes, reports and documents current observations such as patients' physical condition, emotional response and results of procedures, observe skin conditions, variations in vital signs, personal habits of patients, and listen for clues, obvious and subtle, to patient's reaction to illness and treatment which have relevance to the health status of the patient.
o Take Vital signs, perform assessments and report any changes to Head Nurse or on call Provider o Obtain labs as needed.
o Monitor and observe when there is a need for and initiates emergency measures in life threatening situations ( if deemed appropriate due to the environment, there ae times that we do not act, example when there is a DNR- during these times I am the POC to make sure Patient and family is as comfortable as possible and provide emotional support during the transition process)
o Distribute medications to patients per protocol
o Assist with daily feedings, dressing of wounds and any other duties that are necessary to fulfil the mission of the team.
