As the RN reviewer for Case
Management. I was
responsible for reviewing
case manager information
given for a MEDICAID waiver
program. This position was
pending accreditation and
was a new position for
nurses within the agency. I
was involved heavily in
creating the protocols and
procedures for this position
as well as with the case
management documentation
in order to implement future
improvements to clients in
need. I made contacts with
physiciansʼ offices and
assisted in streamlining the
care for patient in the home
following hospitalizations
and transitions.

As a nurse at SAMC, I was

medicating and overall care
of patients on a floor which
had a large population of
dialysis patients and total
care patients; I was
responsible for collaborating
with other members of care
teams such as physicial
therapy and dialysis unit as
well as the physicians and
other nurses (such as
wound care) to provide
excellent care for these
patients. Pain Management
Clinic required me to obtain
vitals and history for patients
in the clinic and assist in
procedures such as pump
refills. Assisting these
patients with their
medications in order to
better control their chronic
pain was essential in helping
to lessen their pain resulting
in improvement their overall
quality of life.

multi-disciplinary team to

patients with multiple
diagnosis. Total care
patients are included with
treatments to include but
not limited to, IV antibiotic
therapy, TPN, wound care,
administering and
continuously monitoring vital
signs per bedside telemetry
monitoring, wound vac
maintenance and overall
care for these complex
patients. Acuity ranges and
the nurse to patient ratio is
5g1.
