Family Nurse Practitioner July 2013 – Present

Contra Costa County Health Services, Martinez, CA

Provide primary care in family practice with a panel of 800+ patients newborn to geriatrics. General health practice of preventative and medical management of chronic conditions, health maintenance, evaluation, diagnosis, treatment, and referral to specialty care. Educate patients about their disease process and how to manage everyday life with their medical issues. Work in general coverage for other providers in the system. Provide substance abuse education and group therapy for opiate dependent patients. Screen for mental health conditions and treat or refer to higher level of care if necessary. Additionally work shifts at CCRMC in short notice primary care/urgent care clinics for all ages, specialty in pediatrics.
