Collaborate with oncologist for complex treatment plans. Obtain relevant health history, complete comprehensive physical exam, order appropriate laboratory and radiologic studies. Provide in depth education of chemotherapeutic drugs and side effect management for patients who are going to receive chemotherapy/immunotherapy. Identify psychosocial needs of patients/ families and make appropriate referrals when needed. Also provide symptom management by prescribing appropriate medications for side effects experienced from chemotherapy, as well as management emotional and spiritual needs.

Director of Inpatient Care Facility, Hospice of Huntington, West Virginia November 2018- October 2019

Responsible for the overall planning, delivery, and evaluation of quality hospice care. Ensure the facility is operating within all state and federal regulations, work with the hospice home team to admit home patients and hospital patients to the facility. Other responsibilities include: the health and safety of the facility, scheduling and supervision of the staff and compliance with state and federal regulations, maintaining a good working environment for staff, patients, families, and community members and partners. Admit hospice patients from home or other medical facilities, provide the appropriate comfort measures according to the patients plan of care, and administer medications ordered by hospice physician to keep the patient comfortable during their final days of life.

Follow patients progress during hospital admission with acute episode of their chronic illness. Facilitate the patients’ navigation through the acute episode to an appropriate transition plan within 48 hours of discharge. Create a plan of care with the patient/family member/provider by transitioning from inpatient setting to home, visit them in their home within 48 hours after discharge to ensure they have what is needed to transition from hospital to home.
