My work with Optum Complex Care Management involves working with elderly patients living in skilled nursing facilities and long term care homes. The focus of my role is ensuring our patients receive the highest quality of care in their current skilled nursing setting, preventing emergency room and hospital admissions. Responsibilities: Assessing, diagnosing and treating chronic and acute medical conditions, ordering and interpreting diagnostic studies, ordering oral and parenteral antibiotics as needed, managing insulin and Coumadin regimens, ordering and renewing prescription medications, writing wound care orders and ensuring proper documentation in the electronic medical record.
