I commenced my anaesthetic training at the Borsod-Abaúj-Zem- plén County Hospital & University Teaching Hospital, Miskolc, Hungary 2006. This is a major teaching hospital with around 1500 beds. This acute teaching hospital is one of the largest public hospitals in Hungary; apart from cardiac and thoracic surgery, every surgical speciality can be found here. Depending on the actual specialty, the hospital provides full medical and surgical cover for 400,000 – 2 million people; it is a regional trauma and burn centre, either. In more than 20 operating theatres, an annual av- erage of 15,000 operations is done, including a large number of emer- gency cases and major surgeries. I covered operating lists for every spe- ciality of the hospital, including neurosurgery, vascular surgery, endocrine surgery, interventional radiology.

In March 2007 started to work part time in medical attendance cen- ter as a G.P. mainly for financial reasons. This center is well equipped with instruments used in advanced life support. In Hungary, and actually in Europe Primary medical attendance is provided by this public facility. For example if acute myocardial infarction is discovered by electrocar- diography vein cannulation, primary anticoagulation, coronary dilatation- al drug, and oxygen therapy is given. Then the patient is transported by ambulance to the cardiology department. I enjoy this part time job, and I get big experience, which was really useful to my daily job. 5. PREVIOUS EMPLOYMENT

6. ANAESTHETIC TRAINING AND EXPERIENCE

7.1 Major General Surgery

I am very experienced in anaesthesia for major general surgical procedures including upper and lower GIT cases, advanced laparoscopic surgery

(laparoscopic adrenalectomy, colectomy, fundoplication etc.) and thyroidectomy. I have experience in both inhalational and intravenous anaesthesia as well as fiberoptic intubation. I utilize invasive monitoring and perform thoracic epidurals routinely for major surgery. 7.2 Orthopaedics and Trauma

I confidently perform regional and general anaesthesia for a variety of elective and emergency orthopaedic surgical procedures. I have performed a large number of spinal and epidural blocks for lower limb and hip surgeries and a wide range of ultrasound guided, nerve stimulator and surface based technic peripheral nerve blocks of both the upper and lower extremities. I am experienced in managing major trauma patients as part of or as leader of trauma teams.

7.3 Anaesthesia for Day Case Surgery

I am familiar with all aspects of day case anaesthesia including suitability for day case surgery, anaesthetic goals, prevention and management of pain and PONV and discharge criteria.

7.4 Obstetrics and Gynaecology

I am very competent managing both elective and emergency caesarean sections under spinal/epidural or general anaesthesia and also managing labour epidurals. I have anaesthetised patients for all types of gynaecological operations including minor procedures, TVT and surgeries of major gynaecological malignancies.

7.5 Anaesthesia for Urology

I am confident managing all types of urological surgery in children and adults, including percutaneous stone removal, TUR of bladder tumour and prostate, open and laparoscopic nephrectomy, radical prostatectomy. 7.6 ENT Anaesthesia

I am confident with shared airway cases and anaesthetized a lot of children for tonsillectomy/adenoidectomy. I am also experienced in anaesthetic management for major ENT malignancies.

7.7 Vascular Anaesthesia

7. SUMMARY OF EXPERIENCE

I have anaesthetised a lot of high-risk patients for numerous vascular surgical procedures including open and endovascular AAA repair and carotid endarterectomy. I am experienced in intraoperative use of cell saver machine.

7.8 Neurosurgery and Interventional Radiology

I am experienced anaesthetising for major neurosurgical procedures such as cerebral aneurysm clipping, tumour resections and spinal surgery including managing unstable fractures. I have experience in anaesthesia for a range of radiologic interventions e.g. cerebral aneurysm coiling, carotid stenting and TIPPS.

7.9 Paediatric Anaesthesia

I am confident with anaesthetizing children for common surgeries in ENT, urology, general surgery, trauma, orthopaedics and ophthalmology. However, I have very limited experience in anaesthesia for infants and special paediatric operations.

7.10 Intensive Therapy

I have worked in well-equipped, level III ICUs in a major acute teaching hospital in Hungary. I am familiar with all aspects of management of the critically ill patient including ventilation and weaning strategies, hemodynamic monitoring, enteral and parenteral nutrition, and ICP management. In Hungary we use PiCCO for hemodynamic monitoring so I have limited experience with pulmonary artery catheters. I have experience in major practical procedures in the ITU including insertion of central and arterial lines, percutaneous tracheostomy, bronchoscopy, chest drainage and chest tube insertion. I enjoy the multidisciplinary teamwork and all the demands that the ICU work puts on me.
