Dear Human Resources:

Please consider a Mastered Prepared Board Certified Nurse Practitioner. As a registered nurse for approximately twenty years, I have extensive experience encompassing broad-based clinical staff nursing in primary care and mental health, as well as correctional healthcare setting (16+ years within the Wayne county jails). Previous, my clinical nursing practice includes primary care assessments and psychiatric assessments, urgent care, occupational health and emergency care. I also have been practicing as a nurse practitioner in home health care for the last six months. My clinical practice as an AANP-certified Adult/geriatric practitioner continues in the vein of my graduate education and clinical experiences (July 2015, Wayne State University). These practicums were directed toward Adult lifespan care focusing on the treatment of acute illness, mental health illness, health promotion and wellness care, and management of chronic disease through client education, support for indicated lifestyle changes, and implementation of appropriate pharmacotherapy. I am familiar in the application of Evaluation and Management, CPT, and Diagnosis coding. Additional skills include computer competence in most Windows word processing programs and office applications and I am comfortable learning informational and clinical technologies. My enclosed resume provides further information regarding my professional experiences. References are available upon request.

I would appreciate the opportunity discussion regarding my qualifications and the needs of your medical establishment.

I may be reached at 734-***-**** (message enabled). Thank you for your consideration. Sincerely,

Maxine V. Hawk RN, MSN, AGPCNP-BC
