Initiated the Walgreens bedside pharmacy program. This program ensures that at risk patients receive prescriptions and education prior to discharge who previously may have been readmitted due to medication mismanagement.

Collaborative membership includes Middlesex Hospital, and an additional nearly 40 agencies including homecare agencies, long term acute care hospitals, and skilled nursing facilities. Initiatives include training and implementation of INTERACT®, disease specific patient education, fall reduction and anti-microbial stewardship.

The Physician Group Practice (PGP) Transition Demonstration was launched by CMS as an accountable care organization pilot to test whether specific payment arrangements can help groups of physicians provide Medicare fee for service beneficiaries with high-quality care coordination and incentivize improved quality, reduced costs and improved patient satisfaction. Middlesex was the only community-based network model among the ten national multispecialty sites selected by the Centers for Medicare and Medicaid Services. Program manager responsibilities include coordination of all aspects of participation for the PGP and subsequent transition demonstration. Role included liaison and oversight of community physician practices, contracts, and leadership for annual abstraction of quality data relative to 32 evidence-based performance clinical measures and audits. Quality data success and reduced expenditures coupled with positive Consumer Assessment of Healthcare Providers and Systems Clinicians (CAHPS) measure of patient experience resulted in the allocation $2.81 MM shared savings payment and $1.77MM in Physician Quality Reporting Incentives (PQRI).
