From my 12 month Evaluation: 2015
Throughout these months, Susan has always demonstrated excellent nursing skills, knowledge of
Department of Aging and Disability Services Regulations, both state & federal knowledge of CMS guidelines for all type of Medicare patients and constantly shows a highly professional leadership style throughout the facility.
In addition to this, Susan always demonstrates total compliance with all state and federal regulations. A very high quality professional DON in every aspect of her position in healthcare.
works extremely well with her Administrators to ensure all is on the same page in the progress of the
facility and its goals to provide the best health care to the residents that is available.
Susan has not only been the DON over the skilled nursing department, but the Assisted Living department
as well. Our facility was surveyed on May 27, 2015 by DADS for our annual Full Book Survey. The
survey results were very good and the facility received a ZERO deficiency rating and in full compliance with DADS.
Thanks to Susan for her professionalism, teaching techniques, nursing skills and hands on training with her staff. She has the personality to bond and strengthen relationships not only with the patients, but with the families as well. Her belief in hands on nursing with residents allows her to meet all required hours of medication administration, and provide that direct care to residents. In short, the most professional nurse
with experience, expertice and knowledge that I have ever worked with.

MISSION STATEMENT
I have always had a deep interest and desire to pursue a career in healthcare. This interest intensified when I was working at the University of Louisville Medical Center. I was most influenced by the nurses that I worked with. I saw how they were the ones who gave nurturing care and more contact with patients. Over the following years, in addition to starting my family. I read and studied whatever was available to me that
would be pertaining to the healthcare field and nursing in general. This was extremely beneficial to me and as a mother as well.
After completing my BSN Degree at LCU University, working as a DON, I then continued to study on my speciality of Geriatrics and to obtain my Nursing Home Administrator’s License. It became my goal to
specialize in this field as this is an area of greatest interest to me and I feel that I can achieve the success I
seek toward planning a career in the area of Long Term Care. This area is greatly neglected, in part due to
the specialization in other segments of healthcare, and to the vast majority of medical personnel who want
to work in hospitals that are located in the metropolitan venues, leaving the other areas void of excellent
care that is really needed. The opportunity I’ve always reached toward is to fulfills the desires that I need to deliver quality skilled nursing care to not only the patients, but the families as well and to delivery those
services on a more personal level. It enables me to help continue and create an atmosphere of continuing care and the compassion for this work.
There are several areas that I have achieved success in which have been very satisfying. The top priority in
my life has been and currently is, that of my family. I have a very strong marriage of 35 years, 3 wonderful
adult children. Two out of school and working and one still in school. I have been blessed with having the
freedom of staying home with all my children throughout the beginning and most critical years of their life.
Another source of accomplishment is my music. I have studied piano and voice for many years, and have
been involved with several vocal and instrumental groups. This is a creative outlet and interest that I have
and will continue to pursue as time allows. My last is the major understanding of that I am so pleased with, and that is having the desire and will to continue with my education and following the career path that
I have chosen in healthcare. Balancing family, work, education, studies and life can be fun.
Doctor Nuland achknowledges the book of Job in the title of his book. Job 18:36, “Who hath wisdom in the
inward parts?” Or, “Who hath given understanding to the heart?”. In the original Hebrew, the translated
word as “heart” is Sechvi. This term is distinctive in that this is the only place in the Bible where it occurs.
There has not been a precise definition for the term, but many rabbinical scholars say that the equivalent is
“mind”. This most certainly acknowledges Aristotle’s belief that the heart is not only essential maintaining
life, but it is also the seat of the mind as well.
The context of this text explores the complex compostion of the body and the intregral chemical reactions that are occurring constantly. Language Doctor Nuland uses is both clinical and poetic. He presents such
a vivid extraordinary and very clear observation of our human form.
The main theme I realized was how every function of the human body works on an independent basis and
that every cellular level has a distinct peculiarly unique only to that particular form.
As a nurse involved in the health care field, I hope to see the patients and their families, not only as a host of symptons, but rather also hosistically. Through this book, I have realized how Job must have felt when
he asked his question of the wonderous wisdom given within each of us.
My Creed Today is:
I have sensed the need,
I see the opportunity,
I feel the call,
I’m going to answer it!
