My primary duty was to conduct home visits with an LPN after certain patients were discharged from the hospital to make sure they had all of their medications, fill their pill boxes, and to make sure they were not having any problems with taking or getting their medications. In the hospital I was responsible for medication reconciliation, which was matching our records with the patient's pharmacy records and making sure there were no discrepancies, scheduling follow-up visits with doctor, and scheduling our home visit appointments.
