Coded both professional and facility charts, with most of my tenure spent with facility charts for multiple hospitals ( large to small) and clients, following all guidelines specific to client, Medical Management Resources, CMS, and commercial payers.
Coded infusions/injections, critical care, fracture care, conscious sedation, wound repair, burn coding, anything that would occur in an emergency dept. setting utilizing all ICD 9 diagnosis and procedure codes as well as CPT codes with correct modifiers.
Programs were varied at this position, as we used each hospital’s program; all were different, and at the time never noted them. Meditech for code entry, utilized NCCI edits for coding compliance. We used client specific grids for E/M code selection.
Attended and passed 2 week facility coding training course required at MMR, for hand- picked coders immediately following 4 week physician coding training with a 98% final score.
