Used hospital data system called CPRS to appropriately gather data pertaining to homeless veterans. The system contains prior psychosocial analyses, all medical, dental and vision records, as well as all of the veteran’s history which includes family, employment, residency, and legal history. This data was analyzed and used to report trends to the treatment team and to recommend appropriate resolutions to pending program situations that were actively involved in the risk assessment process with respect to developing a new treatment plan for successful reintegration into the community for each of the homeless veterans residing in the homeless domiciliary.

I made weekly contributions as a member of the treatment team for all veterans in the homeless domiciliary. Each member contributed any and all information that pertained to an individual veteran. This information was related to the specialization of each team member and could also be as simple as an observation made during the day. I was also responsible for completing a discharge assessment for each veteran that had successfully accomplished the goals of their individual treatment plan. A discharge assessment consist of ensuring that the veteran has seen every staff member that is on their list which is done by verifying that all of the staff members have signed their paperwork which indicates that the veteran has met all of the requirements needed to leave the domiciliary. The discharge assessment also includes conducting a final suicide risk assessment. I also assisted the intake social worker when several veterans were admitted to the domiciliary at the same time. This consisted of completing psychosocial assessments, initial suicide risk assessments, organ donation forms, and mental health advance directives. Mental health advance directives describe what steps will be taken if the veteran is unable to make proper decisions, who the veteran wants to make decision pertaining to their care, if they want to be revived or want fluid transfusions, and burial plans.
