Caregiver for an 84 years old woman in her daughter’s home. January 2014 to April 27, 2015
I performed the following duties.
• Assisted her with activities of daily living including: bathing, dressing, grooming, and toileting
• Transferred her from her bed to her wheelchair and got her to and from activities and meals as requested by family
• Served her meals in her room when she refuses or is unable to join the family for meals
• Recorded and reported changes in her as requested by her daughter who is a medical doctor. I took and recorded her blood pressure and pulse every morning.
• Measured how much water she drank because she is on dialysis
• Provided direct care to the client and range of motion exercises as demonstrated by the therapist
• Was always helping her and supervising her all times to make sure she does not fall
• Accompanied her 3 times a week to the dialysis center for companionship and comfort during dialysis

I perform the following Duties
• Carry out personal hygiene procedures for patients, such as: baths, oral hygiene, skin care, hair care, back rubs, foley care and decubitus preventive care
• Serve meals; assists with feeding if necessary; determine amount of food and fluid intake
• Measure fluid intake and output and record them in the computer
• Make the bed and necessary linen changes.
• Dispose of trash properly
• Take temperatures, pulses and respirations, blood pressures, and record them accurately in the computer
• Answer patient’s call lights and leave call light within the patient’s reach
• Notify the Registered Nurse or LVN of anything in the patient’s condition that requires attention
• Perform any other services assigned by Charge Nurse according to accepted procedure
