WORK EXPERIENCE
Military OneSource Triage Consultant

Responsibilities
Serving as a phone triage consultant in a 24 hour EAP program for active duty military members and their
families.
As a consultant my duties include conducting brief screenings for non- medical counseling, managing crisis
calls and mandated reporting issues as they arise, assisting service members and their families with resource
linking and problem solving with anything that may arise.
School Social Worker
IMAGINE PENN HILLS CHARTER SCHOOL - August 2013 to June 2014
Served as the School Social Worker in a Public Charter School. Provided all Social Service Supports
As the School Social Worker duties include emotional support, resource supports, participating in child study
and assessment and referral for Special Education. School SW also provides group and individual supports
to students on a regular basis. As the Social Worker duties also included truancy monitory and enforcement,
transportation assistance and community supports and activities.
Behavioral Specialist Consultant/Mobile Therapist- MSW
FAMILY BEHAVIORAL RESOURCES - December 2012 to July 2013
Served as the lead clinician within a Family Based level of care. Leading treatment and supervising Bachelor
level support staff.
As the BSC/MT duties included, assessing client needs related to mental health and spectrum disorders
and creating treatment planning and programming to assist client with goal attainment. BSC duties also
included supervision of TSS clinicians in implementing programming developed by the BSC. Consultation and
liasoning services also provided to assist clients in any number of needs. MT duties included in-home therapy
and treatment planning as well as family therapy and treatment recommendations. Both roles included data
collection and evidenced based interventions as well as monthly treatment reviews and authorization review
hearings.
Social Service Coordinator
SOUTHWOOD PSYCHIATRIC HOSPITAL - May 2012 to December 2012
Managed inpatient therapeutic treatment for child and adolescents from ages 4 to 18 from admission to
discharge.
As SSC completed all initial bio-psychosocial evaluations as well as developed inpatient treatment plans and
provided all individual and family clinical sessions. Maintained all contact with outside services and family
members. Assisted psychiatrist with treatment recommendations in daily treatment meetings. Responsible for
all aftercare recommendations and referrals to aftercare services.
Social Worker- MSW
ANOVA HOSPICE AND HOME HEALTHCARE - September 2011 to May 2012
Provided emotional support and resources to individuals and their families while facing the difficult issues of
end of life.
Act as the Social Services director of all services for both the Hospice and Home Health division of Anova
Home Healthcare. Duties include assessment and referral, emotional support, individual, couples and family
counseling at an as needed rate. Linking individuals with any and all services needed to meet their varying
needs. Assist in educating and instilling mental health and behavioral health experience into the agency
through trainings and ongoing collaboration.
Individual Therapy Internship
STAUNTON CLINIC - June 2009 to March 2011
Completed 720 field practicum hours as a concentration requirement for an MSW through the University of
Pittsburgh
As an intern at Staunton clinic job duties include, completing initial diagnostic assessments and creating
individually tailored treatment plans to address varying levels of psychological problems. Weekly to biweekly
therapeutic sessions held and reviewed weekly with clinical supervision. Other duties included completing
session billing notes for chart compliance and 90 treatment plan reviews.
Justice Related Support Coordinator
HUMAN SERVICE ADMINISTRATION ORGANIZATION - September 2008 to September 2010
Provided court and probation liaison services as well as case management assistance to 40 clients involved
in the Alleghey County Legal System
Act as court liaison for a county mental health program that elicits special permission from the courts for mental
health treatment in sentencing. Completes all assessment and referral duties for clients in regards to mental
health and drug and alcohol treatment. For all community clients's acts as probation liaison assisting probation
officers with ensuring compliance with special provisions of probation. Acts as case management for mental
health client's court ordered to treatment by connecting them to funding, housing, transportation and basic

Learn and assist all the daily workings of a Specialize CRR group home from daily processing treatment skills
to regulating of medication distribution to community socialization. As a part of placement learned to develop
a person-centered individual treatment plan for a resident of the group home.

Intake and Assessment Counselor
THE MEADOWS PSYCHIATRIC CENTER - September 2005 to August 2007
Served as intake counselor for 2 yrs in an acute inpatient psychiatric hospital; as well as phone crisis for six
counties and face to face mobile crisis for Centre County.
As an intake worker, answered intake calls, which entailed collecting referral information, reviewing
concerns with nursing, reviewing clinical information with the physician for appropriateness and completing
precertification. Evaluated level of care for walk-in individuals, made appropriate referrals to various programs
and providers; acted as county crisis, calmly deescalated individuals in crisis situations both face to face and
over the phone; performed as shift supervisor, managed the flow of referrals into the hospital, assigning duties
