ABOUT MY EXPIRIENCE IN “CARDIAC ANAESTHESIA ” (Training In India)
I am the “First Cardiac Anaesthesiologist in Pune, India” and the second only in state of Maharashtra to be certified with degree by the National Board, New Delhi to practice Cardiac Anaesthesia.
I was one of the 8 candidates selected (every year) by all India level entrance examination conducted by the National Board, New Delhi for 2 years training in cardiac anaesthesia in a highly specialized centre in the country. I have completed my training from Madras Medical Mission, Chennai which is one of the premier and reputed institute for cardiovascular diseases, where the first heart transplant was conducted in the country. The institute performs about 2500 open heart surgeries a year, half of which are of pediatric group. The institute also runs training and research programs in the field of cardio-vascular specialty.
I have passed my “Cardiac Anaesthesia” degree examination conducted by the National Board, New Delhi in first attempt (2006) on completion of my 2 year training in the institute.
During my training I got an opportunity for giving anaesthesia and managing variety of patients coming for cardiac surgery which included:
1. Pediatric Cardiac : Simple & Complex Congenital Corrective Surgeries for Neonates &
Infants.
2. Adult Cardiac : Patients coming for Routine and High Risk
Coronary Artery Bypass
Valve Replacements / Repairs
Aneurysmal and Dissection surgeries of Aorta
Thoracic Surgical Cases.
Other Rare Cases like Mass Excision, Pulmonary Embolism etc.
3. Intensive care Unit : Training in managing the Post Operative Period of patients in
Post- operative and Cardiac Intensive Care Unit.
4. Teaching : Academics of Department and Training of Paramedical Staff,
Residents and Medical Students.
5. Managing Emergencies in Cath-Lab Procedures and In other Remote Hospital Areas.
Prior to training in Madras Medical Mission, I was working in SVIMS super speciality institute,Tirupati where I was one of the 2 candidates selected by all India entrance examination (every year) for post doctoral training in cardiac anaesthesia which is recognized by Tirupati university and Medical council of India. I had a good exposure for giving anaesthesia for adult cases mainly valve replacements, coronary artery bypass grafting and thoracic lung cases.I also had the opportunity to give anaesthesia for noncardiac surgical patients with cardiac disease.I was also involved in the training of post graduate residents and paramedical staff during my stay in Tirupati.
Prior to taking up any certified course I was working in Deendayal Hospital, Pune as a junior consultant in Anaesthesia and Critical Care where I was involved in doing cardiac as well as non cardiac cases and also taking care of Intensive care unit.
ABOUT MY EXPIRIENCE IN “CARDIAC ANAESTHESIA ” (Training Abroad)
I completed a Two year “Fellowship in Cardiac anesthesia” at “Montreal Heart Institute, Canada” where about 2000 open heart surgeries are performed annually.
This is Canada’s Most Premier Institute of Repute for Cardiac Care, which is more than 60 years old and has the honor and credit of performing many of the cardiac procedures for the “First time in North America”.
Here, I had my Advanced cardiac training program with exposure to:
Trans-esophageal echocardiography(Intra-Operative Assessment and Reporting)
Echo Guided Invasive Monitoring Lines
Advanced Cardiac Monitoring and
Academic programs and Teaching
Of my 13 years of experience in Anesthesia, 10 years of my experience was involved in cardiac work. I have been involved in giving anaesthesia for more than 3000 adult cardiac surgery cases,200 pediatric cases and 300 cardiac catheterization procedures(diagnostic and interventional) which includes a wide variety of complex and high risk cases.
About my work experience in Srilanka:
After completing my Fellowship in cardiac anesthesia from Canada I took assignment as a consultant in cardiac anesthesia to develop the existing cardiac surgical services at Durdans hospital, Colombo, Srilanka which is the most advanced and sophisticated cardiac centre in the country. Annually they perform about 800-1000 open heart surgeries. I had the opportunity to implement the knowledge and expertise to perform Trans Esophageal Echocardiography for cardiac surgical patients and also teach the staff about it. I also got to implement current changes in Intensive care management in Coronary care unit and cardiac surgical intensive care unit. I also began my research projects related to cardiac surgical services.
Experience in cardiac care setup establishment and services :
I was privileged to have been assigned a project by Apollo Lanka Hospitals, Colombo to make a project and analysis of cardiac care set up establishment and services. It was in depth analysis to make a study on existing services and provide road map to enhance the cardiac care services.
I also took up a similar assignment in my country to establish a care centre in Ludhiana and also helped the cardiac surgery unit to be established by performing and taking care of cardiac surgical patients. I was also assigned to train the staff in the hospital so as to enhance the level of patients care.
Experience in Current job :
I addition to job assigned as a cardiac anesthesiologist, I am also taking care of cardiac coronary care unit where all types of complex cardiac patients with APACHE score > 25 are admitted for intensive care management under my direct care and supervision.
Procedures performed independently :
• Intubation and Airway Management
• Mechanical Ventilation strategies and use of various modes in patient management
• Invasive Arterial line & Monitoring
• Central Venous line & Monitoring
• Pulmonary Artery Catheter & Monitoring
• Intra Aortic Balloon Pump & Monitoring
• Trans Venous Intra Cardiac Pacing
• Trans Thoracic and Trans Esophageal echocardiography
• Ultrasonic guided invasive vascular cannulation
MY FUTURE JOB PLAN:
I am highly interested in pursuing my career in the field of Anaesthesia (Especially cardiac) for which I underwent the certified training in India as well as Abroad. I am interested in developing the Department of cardiac anaesthesia wherever I am given an opportunity to do so. I have also the interest to impart knowledge about the field of cardiac anaesthesia and intensive care to the postgraduate students and the paramedical staff. I am interested in pursuing research work, presenting and publishing articles related to cardiac anaesthesia in conferences and international journals. Given an opportunity I am sure that I would actively participate in the development of cardiac anaesthesia department in the institute.
