Helped create and refine a tool for residents spending time out of the country to co manage
high risk patients during PCP absences. Included identifying needy patients, creating
problem list with goals, identifying patient’s community and family connections and barriers
to care. Goal to improve chronic disease management, reduce ED visits and use of
unfamiliar providers minimizing repeat lab work, imaging, etc.

Designed and implemented a continuity of care program for high risk patients referred in
for intensive RN follow up by providers in the clinic. Called and tracked patients, updating
the referring providers and the electronic health record. Results included a >50% reduction
in the use of ED by patients in the time period tracked, as compared to the same time
period before entering the program. Individual patient improvements included better
adherence to medication use, improvements in blood pressure, glucose monitoring and
weight maintenance as well as more follow up with specialist appointments.

Participated in a weeklong medical brigade setting up a clinic for two days at two different
sites. Organized medications, labeled in Spanish and with pictograms, and performed triage
and discharge instructions. Helped to found and organize the group's mission for this
continuing project.
