Working to correct a downward trend in staffing, resident care, family satisfaction and survey corrections. This
1 Star building was in serious trouble and I spent five months bringing stability and hiring quality staff. They
are well on their way to great resident care and better survey outcomes.
Treemont Healthcare and Rehabilitation, Dallas Texas July 22, 2013 to the December 2013
Oversight of a 130 bed skilled nursing unit with an average census of 15 Medicare, 5 Managed Care and 65
Medicaid. Supervised the conversion of an entire wing into private rooms to enhance our Medicare short-stay
residents. We had a zero deficiency survey and are rated 5 Stars.
The Legacy at Willow Bend, Plano Texas May 24, 2013 to July 15, 2013
Served as their interim administrator over their 40 bed skilled nursing center. They were in between
administrators and were awaiting the hiring of a new Executive Director over their CCRC, so they only needed
an interim. I worked on organizing their Survey Prep Book, updating their contract notebook, reviewing charts
and doing audits on consents, getting their management team on the same page and working with the residents
to bring some calm and stability.

Oversight of The Plaza, with 72 skilled beds, 60 Assisted Living and 31 Memory Care apartments. Census is
averaging 98-100 %, staff turn-over is small, satisfaction scores are great and state surveys have been positive
with zero deficiencies.
