Billing Knowledge
Billing and Coding:
*Charge Entries (ICD-9, CPT codes, HCPCS codes, Modifiers, Referral number, Pre-certification and
Authorization number).
*Scrub claims for demographic, insurance and coding errors and submit claims electronically to clearing
house and by HCFA 1500 to primary, secondary and tertiary insurance with EOBs.
*Download clearing house reports for rejected claims and correct rejected claims and resubmit corrected
claims. Contact clearing house for batch rejections and unresolved errors. Contact Practice
Management Vendor for software update requirement and unresolved issues.
*Knowledge of CPT, HCPCS, ICD-9, Modifiers, HCFA 1500, and Medical Terminology.
Insurance claims:
*Detail knowledge of EOB (explanation of benefits) and denials codes from insurance.
*Contact insurances for payment status of claims filed electronically (20 days limit) and paper (35 days)
through phone and provider portal.
*Dispute claims denied in error and have it send for reprocess for payments.
Aging Report:
*Generate Aging reports weekly and follow up on claims from old date of service to current and larger
balances to lower balances.
*Correct claims denied by insurance and resubmit them electronically or by paper or fax with supporting
documentations.
*Appeal claims denied in error by phone and written request with supporting documentations. Request
interest from insurance for unpaid clean claims not paid within Texas Insurance Law time limit.
*Contact referring physicians and patient for claims denied by insurance for pending information.
Patient Account:
*Send out patient statements electronically and by paper on regular intervals (Bi-weekly or Monthly).
*Work on rejected statements received from clearing house and return mails.
*Assist patients in understanding account balances (copay, coinsurance, deductibles, coordination of
benefits by primary, secondary and tertiary insurances).
*Collect payments over phone and in office. Assists patients in paying large and old balance through
payment plan agreements.
*Generate patient Aging reports and contact patients through collections letters and phone.
Payment Posting:
*Post electronic payments (ERA: Electronic remittance advice) and paper EOBs (Explanation of
Benefits) from insurance.
*Post patient payments (credit card, money order, checks and cash).
*Reconcile, balance and prepare end of the day batch deposits.
Financial Reports:
*Generate Practice Analysis reports, Monthly, Quarterly and yearly financial reports,
*Insurance and Patient Aging reports, CPT frequency report, E & M visits reports, etc.
Others:
*Insurance verification and benefits, obtain referrals and authorizations, Credentialing knowledge,
Employee training, Data Entry, and Front office Managing.
