• Manage ongoing caseload of up to 80 clients with regular home visits with assessment and re-assessment of
clients' ongoing needs. Help and screen clients’ for Medicaid, Food Stamps, Meals on Wheels, and Senior
Citizen Rent Exemption.
• Follow NYC Department for the Aging regulations and ensure that clients remained safe in the community
by providing them with all of the appropriate services.
• Make monitoring calls to all clients at least once a month and follow-up on individual care plans.
• Keep record of all interactions with each individual client and maintained all of client's files up to date
• Refer clients to recreational programs.
