PROFESSIONAL SKILLS

Overall

During my 7 years of employment in the Dept. of Gastroenterology Central Teaching Hospital of

the Medical University of Silesia in KatowiceI I performed daily doctor's duties based on good

clinical practice rules and regulations. I was responsible for the well-being of the whole duration of

the inpatient stay, from the time of admission to the time of discharge. There was usually and

average of 8-15 inpatients at any one time. I had the responsibilities of ordering any and all

investigations required including blood investigations, Xrays or endoscopies. In addition with prior

consultation with Senior Consultant and/or Chief of Department, I am responsible to plan and

actualise the therapy. I had to do simple diagnostic endoscopies and care for the patients after

therapeutic procedures. I was responsible for all medical documentation during their stay in

hospital as well as post-discharge.

On a monthly basis I had to have at least four 24h(overnight) shifts when I was responsible for all

the 75-100 beds in the Unit. During these shifts I had to assess and treat any new admissions into

the Department (1-5/ night) because we had Emergency Admissions for acute pancretaitis,

cholangitis and GI tract bleeding. I was also responsible to provide ALCS (advanced life cardiac

support) in sudden cardiorespiratory collapses.

During my 1,5 year stay in Endoscopy Division Policlinico Gemelli in Rome I assisted in overall

patient care and wellbeing during and after the endoscopic procedures as an internal physician,

participated in clinical working rounds, assisted in endoscopic reporting and data collection. I am

the primary operator for regular weekly lists for diagnostic and therapeutic gastroscopy and

colonoscopy. Cases include polypectomy (more than 50 polipectomies bigger than 1cm), ligation,

hemostasis (APC/Argon plasma coagulation, uni/bipolar coagulation, hemoclips, injection

therapy).

I have administered analgesia and sedation during endoscopic procedures and take care about

patient during recovery.

During my over 3 years of employment in Dept of Internal Medicine and Gastroenterology in

Central Clinical Hospital Ministry of Inferior I was responsible for the well-being of the whole

duration of the inpatient stay, from the time of admission to the time of discharge. Working in the

Endoscopy Unit I have done by myself more than 500 ERCP, 300 EUS and over 500 gastroscopy

and colonoscopy (diagnostic and therapeutic). I was response for on call duties (interventional

gastroscopy in cases of GI bleeding) and overnight shifts in Department.

For over 2 years I am independent Endoscopist and Consultant in Gastroenterology in two biggest

private healthcare network Luxmed Bupa Group and Damian Medical Centre Medicover Group. I

am the primary operator for regular weekly lists for diagnostic and therapeutic gastroscopy and

colonoscopy. I do professional consultation in Gastroenterology for outpatients in these network.

Endoscopic Retrograde Cholangiopancreatography (ERCP) 500 procedures

As first and second endoscopic operator in both simple and complex pancreatobiliary cases which

include common biliary calculi and post-liver transplantation strictures, malignant strictures; as the

dell'Unità Operativa Endoscopia Digestiva Chirurgica del Policlinico A.Gemelli is the main

endoscopic referral centre in Italy and also receives complex cases from around the EU.

Echoendoscopy and Fine Needle Aspiration (EUS and FNA) 400 procedures

As first and second endoscopic operator in diagnosis cases of the pancreas, liver and peripheral

digestive tract lesions. If necessary FNA for cytology or histopatalogy examination. Also of note

with specialty focused on intraductal papillary mucinous neoplasm (IPMN).

Audit of completion to caecum data - we have no official statistic - but I do in a week 30 -40

colonoscopies, and I am unable to get cecum 2-3 times a month

How is caecum identified - I work in Dept where we are focus on IBD so we must go through the

Bauhin valve or we must write that we were not able to do it

Adenoma detection rate - nobody counts that in my hospital

Midazolam - average 3mg i.v. max. 5mg i.v.

Pethidne - not allowed in Poland - only for anaesthesiologist - when I was working in Italy we

usually used 25-50 ug in a “step by step” analgosedation

Complication rate - I had only one perforation during ERCP - but it was delayed perforation after

2-3 days, she has undergone surgical operation
