I have currently attending UOP working toward a Substance Abuse Counselor certificate. I chose to
attend this course to continue my education on substance abuse. My completion date was December 2013.
I have done internship at the Recovery House, Service First and the Salvation Army. I worked for El
Dorado House, a live in Drug Treatment Center for ex-felons with drug related problems, and currently
work for Circle of Friends with A-B 109 clients. I have conducted intake assessments to determine client
problems and appropriate course of action, provided substance abuse counseling to individuals, families
and groups, conduct group educational programs, provided crisis intervention, counseling, developed
recovery plans, makes referrals to appropriate service providers, maintain client case records, technical
client charting, participated in meetings, committees, training, planned schedules, implement client’s
daily work, recreational activities, monitor client files, prepared reports for other agencies as required,
monitor ongoing treatment, developed aftercare plans, and provided client follow-up.
I have provided substance abuse counseling work within the community, engaged in a variety of
counseling and prevention activities targeted at individuals, groups, and families experiencing problems
related to substance abuse. My work experience relates to outpatient and residential facility programs.
