-Clinical Review Pharmacist.
This is a closed door facility. My main duties were to review scanned
prescriptions from physicians to assure correct directions, dose, and MD
phone numbers and date written to avoid any medication dispensing errors.
I also reviewed patient profiles when necessary for name, date of birth and
doctor's name and dosing history of the drug in question. This was to
prevent overlap of controlled substances and potential allergic reactions.
This process was done utilizing proprietary Walgreens Computer Software.
10-2011 to 9-2012 Rx Relief Staffing, Fresno, CA /Avella Specialty Pharmacy
-Verification Pharmacist.
This facility is a closed door mail order pharmacy. My duties as contract
pharmacist were to receive medication from all departments that consisted
of Oncology, Fertility, Veterinary, and Dermatology sections in order to
verify correct quantity, drug, and patient's name to prevent any errors
done by the pharmacy technicians. My knowledge of Spanish and English
helped me communicate with the pharmacy technicians from other departments.
This improved processing of orders to insured correct and accurate
results.
4-2010 to 7-2011 MedPro Staffing -Veterans Health Administration, Dublin,
GA
- Staff Pharmacist.
This is an outpatient pharmacy. I processed 100% of all new, mail order,
and partial prescriptions orders prepared by technicians for accuracy in
directions and quantity.
Periodic patient consultation was done throughout the day as needed. As a
pharmacist, I was constantly faced with the use of professional judgment in
creating solutions for questionable prescriptions. Prior to dispensing
medication, drug identification was accomplished using VISTA software to
identify shape, color, and lot number to assure safety in filling the
prescription. This was to avoid errors in medication selection when
filling.
8-2009 to10-2009 AB Staffing -Veterans Health Administration, Vancouver, WA
- Staff Pharmacist.
This is a mail order as well as outpatient pharmacy. I helped increase
productivity by reviewing the pharmacy technicians' methods of organizing
paperwork designated for each patient. This avoided prescriptions errors
and prevented patients receiving another patients' medication. Thus,
established a more efficient and productive process.
1-2009 to 5-2009 CIGNA Health Systems.
- Staff Pharmacist.
This was a floating pharmacist position throughout the CIGNA Network in
Maricopa County. My duties and responsibilities were to consult patients on
their medication. I was also responsible for consulting all Spanish
speaking patients on their medication use. I received, reviewed, and
dispensed all new as well as refill prescriptions.
1-2008 to 7-2008 Medco Health Solutions.
- Staff Pharmacist/Diabetic Specialist.
This was a work at home position. It dealt with direct patient contact via
CISCO telephone systems. My duties and responsibilities were to contact
Diabetic patients and review their medication profile with them on any
questions concerning their Diabetes. I also discussed foot care, weight
management, exercise, and the importance of regular drug maintenance and
home testing of their blood sugar.
Microsoft Outlook and Excel were used for maintaining calls and notes.
Instant Messaging was used to contact other pharmacists. Physicians were
contacted on any questions concerning medication for their patients via
telephone.
3-2007 to 1 -2008 On Assignment Healthcare Staffing- Womens' International
Pharmacy.
- Staff Pharmacist.
This is a mail order specialty pharmacy which dealt with veterinary
products as well as hormonal therapy for women. I was responsible for
working with pharmacy technicians who filled veterinary prescription
orders. I consulted patients on the use of their medication for their pets
and possible side effects. Veterinarians were contacted via phone on any
questions concerning medication for the pets.
4-2006 to 2-2007 Pharmerica
- Staff Pharmacist.
This is a close door specialty pharmacy. Services were provided to long
term nursing facilities and high risk at home pregnancy programs. My
duties were to verify all new and refill orders filled by pharmacy
technicians. Emergency medical boxes were checked for correct medication
upon refilling. I assisted with on call duties to take care any afterhours
medication needs the nursing homes may have requested.
9-2000 to 12 -2005 Maricopa Integrated Health Systems
- Staff Pharmacist -Float Pharmacist.
This was a floating pharmacist position for Maricopa County Integrated
Health Systems. I covered all satellite county clinics with pharmacies.
Duties and responsibilities involved opening and closing the pharmacy at
the end of the work day. I was responsible for reviewing all the written
prescriptions by in-house physicians for accuracy. Patients were consulted
in English and Spanish on their medication use and physicians were
contacted on any questions concerning the patient's medication.
Skills:
Fluent in Spanish, experience with Script Pro, Pyxis Med Station, Microsoft
Outlook, Word and Excell. Good interpersonal communication skills.
