CLINICAL EXPERIENCE:
Clinical rotation sites at Mercy Mt. Airy Primary Care, Mercy Monfort Heights, Mercy Delhi,
and Mercy Addyston locations. The patient population consists of underserved, middle class to
upper class. Clinic sizes are approximately 2,000 to 4,000 patients (1 to 4 physicians [depending
on site], 1 NP) with a patient focus of adult and gerontology. I performed assessments for
depression utilizing Becks screening tool and treating depression. Total clinical hours at these
locations were 384.2 hours, with 10 patients seen independently daily and followed up by NP
preceptor. I became proficient with electronic medical records (EPIC), ordering and interpreting
labs, and other diagnostic testing and writing prescriptions.
Clinical rotation site Cincinnati Health Department (Braxton CANN Clinic) is located in
Madisonville, Ohio. This clinic serves at risk lower socioeconomic population. The size is
approximately 3,000 patients (1 physician, and 1certified nurse midwife [CNM]) with a patient
focus of women’s obstetrics and gynecological healthcare. The total clinical hours at this
location was 105 hours, with 8 patients seen independently daily. I became proficient in pap
exams, slide preparation and wet mounts, sexually transmitted infection screenings,
interpretation of findings, Medicaid, Medicaid HMO, and community resources.

Clinical rotation site at Mercy Medical Associates is located in Kenwood, Ohio. Population
serves middle to upper class patients. Clinic size is approximately 1,500 patients (1 physician
and 1 nurse practitioner [NP]) with a patient focus of family care (pediatrics, adults and elderly).
Utilized DSM IV to assessing adult/children for ADHD and evidenced based guidelines for
treatment. The total clinical hours at this location was 174.5 hours, with 10 patients seen
independently daily by the end of this rotation. I became proficient with cryotherapy.

EMPLOYMENT:
Cincinnati, OH Position: RN (Hematology/Oncology Clinic).
Responsibilities include patient assessment, administration of blood
products, chemotherapy in the outpatient clinic setting, and electronic
medical record documentation (EPIC system).

Position: RN Acting Team Leader (Home Healthcare). Responsibilities
included managing daily operations for maternal/child departmental
Public Health Nursing and Community Health Worker staff. Manage
referrals for high risk antepartum and postpartum clients. Ordered supplies
and oversaw maintenance of supplies and equipment for nursing staff.
Organized and advised Public Health Nurses and Community Health
Workers during care conferencing for high risk clients within the agency.
Orientated new Public Health Nurses regarding the policies and
procedures of the agency, Medicaid, Medicaid HMO, Medicare, self pay
clients, medical coding, EMR (Alscripts) and case management process. I
have built solid trusting relationships with staff and client families,
generating positive PR through extra efforts in care management and one -
on-one communications. I held blood pressure clinics within the
community.
08/2005 to 09/2012: Cincinnati Health Department. Cincinnati, OH
Position: Public Health Nurse. Responsibilities included case management
for antepartum and postpartum women of all ages, pediatrics and
adult/geriatric health population. Patient and family education on an array
of health care issues. I performed in home assessments for all age groups
and postpartum depression. Facilitated a blood pressure and diabetic
screening program with a local community recreation center while
promoting preventative health care with an at risk population.
Collaborated with multidisciplinary team members, working closely with
physicians, nurses, social workers, pharmacists, therapists to formulate,
implement and modify individual plans of care. Proficient with EMR
