I have the skills and experience to work as a support worker in the care sector. I can make a
real contribution to this role through my range of personal qualities and skills such as the ability

drinks, enable service users to achieve independence as far as possible. Again I have got a strong

Role:
1. Participating in the care and assessment of service user’s problems, planning,
implementation and evaluation of interventions, as required, ensuring that the service
user’s physical, psychological and social needs are taken into account at all times.
2. Delivering effective individualized care in response to service user’s needs under the
supervision of the nominated professional, providing a range of supportive interventions
/ home treatments.
3. Ensuring clear and concise documentation of individual service user care in all service
user’s files and that these are stored according to local policy.
4. Seeking to establish and maintain an effective working relationship with service users,

independence.
