EXPERIENCE
10/17- Current: Medical Social Worker: Alignment Health Care, Santa Clara.
My current caseload encompasses all of our members throughout Santa Clara County. I conduct home visits with individual members. I provide resources and advocacy for the members. I conduct PHQ-9 and Mini Mental assessments. I cooperate with the Care Anywhere medical team to provide the best treatment plan for the members and their families.
6/16-10/17: Investigator/Social Worker III: Human Service Agency, Child Welfare Services, San Mateo.
I worked for the Juvenile Court Unit of the Child Welfare Services. My duties were to investigate cases referred by the Emergency Response Unit. I was to confirm if the findings were accurate and to validate the testimony of the witnesses in the report. I was in charge of crafting a clinical case plan for the family that would address the issues that brought the family to the attention of the Court. My goal was to create a Jurisdictional/Disposition Report for the Court. That report would confirm the needs of the family and the reasons they are before the Court. The latter half of the Report, the Disposition, addresses what efforts have been made to ultimately justify the recommendation of the Report.
6/14-4/15: First Chance: Star Vista.
Duties were to cover the shift over the weekend for First Chance. First Chance is the sobering station for San Mateo County. My duties as a staff member were to process all clients into the station, secure all valuables, and ensure that the clients are safely treated and kept under care while in First Chance custody. My further efforts involve the clients upon release; an assessment is performed to determine drug/alcohol usage. The intention is to educate the client of the dangers and risks as well as provide resources and the opportunity to change prior to being released.

12/10-12/12: Case Manager for Differential Response: Edgewood Center for Children and Families
6/06-4/10: Case Manager for Differential Response: Youth Family Enrichment Services
Duties were to assist families in crisis in securing local resources. Also included were advocacy for the families in dealing with their issues and how to overcome the challenges in the path of stability. The Case Manger is bilingual and culturally competent, willing to engage families in their homes or meet them at community sites. The services that the Case Manager provides are that of representation, referrals and active counseling. The Case Manager handles a caseload of fifteen families or more. All case notes are recorded in the internal ETO software system from which quarterly reports are issued.
4/10-12/10: Behavior Coach for ISIS: Edgewood Center for Children and Families
Duties are to provide behavior services for youth in the age range from 7 to 17 in relation to therapeutic goals.
Addressed therapeutic goals via home visits/school visits and practicing coping techniques, role-playing and skill building exercises with the youth for measurable outcomes.
7/05-6/06: Bilingual Housing Counselor: Fair Housing of Marin, San Rafael, CA
Responsible for maintenance of database entries regarding housing counseling calls. Test coordinator for a staff of 22 committed testers. Assist with the accumulation of statistics for periodic reports. Outreach to communities.
4/02-6/05: Support Line Counselor: Community Overcoming Relationship Abuse (CORA), Burlingame, CA
Promoted to Support Line Counselor, was responsible for managing a staff of crisis counselors and advocates and supporting other staff members during their shifts. Provided training in English and Spanish to new staff and volunteers on agency procedures. Assigned counseling sessions and participated in shadowing the first five sessions for novice counselors. Compiled the resource guide and updated the guide periodically. Fulfilled overnight shifts. Counselor arranged departmental meetings and gathered client data for statistical reporting to grant funders.
3/01-4/02: Support Staff: Community Overcoming Relationship Abuse (CORA), Burlingame, CA
Duties involved input of all Emergency Outreach Programs, Hotline calls, Legal line referral updates and Counseling session intakes into an Access database. Assist with the case filing and the management of the database.
3/00-12/00: Development Assistant: Planned Parenthood Golden Gate, San Mateo, CA
Assistant was responsible for the organization and consistency as well as accuracy of the donor database. Document the daily entry of cash, check and credit card gifts into the database. Acknowledge all gifts received in a timely manner. Act as a backup database administrator to the System Manager. Create customized reports dealing with solicitor progress, annual fund activity, special events and financial progress reports. Provide the System Manager with support for the production of in-house appeals, including merge processing, assisting with donor segmentation and creating tables for Public Affairs.
