College, Bangalore.
Worked in rotations in the department of Medicine, Surgery, Obs&Gyn, ENT, Ophthalmology, Orthopaedics, Dermatology and Community Medicine.
CLINICAL SKILL
I can perform tasks such as history taking, examination, IV cannulation, blood sampling for ABG, urinary catheterization, Intubation and nasogastric tube insertion Independently. I have also performed procedures such as pleural tapping, insertion of chest drain and needle thoracocentesis under supervision. I can do lumbar puncture. I was involved in assisting the staff in providing various types of Anaesthesia such as spinal, Epidural, Regional and General and resuscitation. I also have maintained airway via intubation and vitals of the patient during surgery. I have been able to perform interventions such as intubation and advanced cardiac resuscitation. In the A&E, I have stabilized patients with trauma while following the trauma life support guidelines.
TEAM WORK
As a team member, I was involved in working in a team assisting the registrar. While in the Accident and Emergency, there was a case of trauma. As the registrar was maintaining the airway, I started an IV line and resuscitated the patient with fluid. While doing this, I found the patient severely anaemic clinically. I brought this to the notice of the registrar and it was decided to start a blood transfusion.
As a team leader, there was an instance for example when I was in the medicine ward when a patient developed chest pain. I administered oxygen, aspirin, GTN and opioids to the patient. I got the paramedics take an ECG. I asked the nurses to record the vitals. I informed the registrar and cardiology team who took over the patient. I informed the consultant and took instructions in management. I counselled the patient’s relatives about his condition.
CLINICAL AUDIT EXPERIENCE
I had the chance to do an audit on the “labelling the iv cannula with time and date” in the ward during my work in Mediscope hospital in Dec 2008. During my ward round, I noticed that the iv cannula on one patient was not labelled with the date and time of cannulation. On further inquiry with the patient, I found that he had the cannula for more than 72 hours. I checked the cannulas of all the 25 patients in the ward and found that 8% of them did not have the iv cannula with time and date labelled on them. The NICE guidelines suggest an IV cannula not more than 72 hours. I presented my audit to the ward team in the weekly ward conference and stressed on the need to label the IV cannula. It was decided to do a re-audit after 2 months.
