1.Family nurse practitioner working in collaboration with Internal Medicine Hospitalist physicians assisting with hospital admissions from the Emergency Department to the medical, surgical, orthopedic/oncology, telemetry floors in patients with various medical conditions such as acute congestive heart failure, atrial fibrillation, supraventricular tachycardia, venous thromboembolism, peripheral arterial disease, COPD exacerbation, Idiopathic pulmonary fibrosis, acute ischemic stroke, tranverse myelitis, Upper GI bleed, ascites, hepatic encephalopathy, alcoholic hepatitis, Clostridium Difficile infection, infectious diarrhea, acute pancreatitis, acute liver failure, IBS, sepsis, septic shock, diabetes mellitus complications, acute kidney injury, thyroid conditions, venous thromboembolism prophylaxis, GI prophylaxis and other acute conditions; developing an individualized care plan, initiating admission orders to include broad spectrum antibiotics when indicated, resuming home medications if indicated, diet, Intravenous therapy or hep-lock, PT/OT therapy, medications, and initializing an individualized plan of care during hospital stay, making daily patient rounds, ordering laboratory testing, diagnostic studies, correcting electrolyte imbalances, collaborating with admitting physician and specialty consultants to optimize patient care and recovery. Collaborating with other health professionals including medical residents, medical students, nurses, physical and occupational therapists, social workers and other physician consultants to provide optimum care and discharge planning to home, home with home health services, Rehabilitation, long term facilities or hospice care. Ethical dilemmas in this area included advocating for the patient who was not ready for discharge when insurance and facility protocol requested patient should had been discharged.

Assessed and treated and average of 15-20 patients a day with common skin conditions within the veteran’s population.
