Nurse Family Partnership is an evidence based, community health program that helps transform the lives of vulnerable mothers pregnant with their first child. Each mother Nurse Family Partnership serves is partnered with a registered nurse early in her pregnancy and receives ongoing nurse home visits that continue through her child’s second birthday.
• Manages 30 Medicaid clients and their children in Greenville, Pickens, Oconee, and Laurens County through biweekly home visits or Telehealth.
• Educates families on prenatal and antepartum care, labor and delivery, breastfeeding, child development, anticipatory guidance, and infant and toddler nutrition.
• Monitors developmental progress through Ages and Stages Questionaries (ASQ-SE).
• Collaborates with community partners such as DSS, Law Enforcement, Safe Kids Upstate, etc to provide patients wholistic care.
