Review and interpret inpatient medication orders.
Monitor labs and drug levels and make therapeutic recommendations.
Screen for suspected adverse drug events (ADEs) and document appropriately.
Provide accurate, comprehensive evidence-based drug information to patients and other health care providers in both written and oral formats.
Interpret laboratory values as they relate to a patient's health condition and drug therapy
Actively participate in daily rounds.
Reporter role, patient’s chart review, grants application for the Quality and Safety annual conference.
Investigating for malpractice based on patient’s medical record in order to fill out the Department of Public Health form as well as the Safety and Quality request form.
Participating in Patient Identification observation ( Inpatient/ outpatient), Hand hygiene observation, Clostridium difficile infection observation.
Receiving and processing request order generated from the Ominicell machine throughout the hospital. Shadowing Pharmacists in order to be familiar with the daily routine of a hospital pharmacist’s work. Participating in narcotics inventory.
Assisting patients over the phone when they need to renew their prescriptions.
Coordinating the Nutrition Program at Partners in Health/Haiti which was based on the government guidelines. Developed and implemented the nutrition system in different branches of the organization. Planning, direction, implementation and training on use of ready-to-use-therapeutic-food for health care providers across multiple clinics in central Haiti.
