I am Masters in Clinical Pharmacy (Coursework) from Universiti Sains Malaysia, having more than four years’ comprehensive working experience in tertiary care hospital (22 months in Prince Salman Armed Forces Hospital Tabuk, N.W.A.F.H, Saudi Arabia, 2 years in Combined Military Hospital Lahore, Pakistan, six months in General Hospital Malaysia as a part of clerkship and currently working as a clinical pharmacist in JCI accredited hospital, Shaukat Khanum Memorial and research Hospital, Lahore, Pakistan). Moreover, I am registered as a hospital pharmacist in Saudi Arabia, Pakistan and currently holding Massachusetts’ intern pharmacist licence, America.
I have been working in all areas of pharmacy operations like inpatient, outpatient, compounding, aseptic area, and cytotoxic department. I was assigned as clinical area pharmacist for different hospital sections like ICU, CCU, PICU, NICU, Internal medicine, oncology, renal, surgery, psychiatry and rehabilitation etc during my working experience.

PRINCE SALMAN

Outpatient and Inpatient pharmacist of different nursing stations including medical, surgical, renal, psychiatric, rehabilitation, paediatric, neonatal, adult, ICU, CCU, Burns.
1. Clinical activities in clinical care area (AICU, PICU, BURNS, CSU, CCU) and in psychiatry unit including ward rounds with doctors, choice of medications, individualization of doses, dose adjustment based on renal and hepatic dysfunction.
2. Worked in IV room, TPN compounding area, and chemo room. 3. Monitoring of lab reports for proper indication of antibiotic, drug- drug interactions, duplication of same class ensures timely delivery of medication to nursing units.
4. Interpretation, evaluation and individualization of doses in pediatrics 5. Dose calculation, TDM, dosage form selection, specialized drug preparation and administration techniques
6. Monthly floor stock inspection for proper storage of drugs. 7. Provide drug and poison information to medical and paramedical staff 8. Dispensing of controlled drugs.
9. Ensure the accuracy of label before dispensing to right patient, right drug and right quantity.
10. Contact the Physician for inaccuracy in order, proper documentation and communication of it to nursing staff.
11. Delivering lecture for nursing orientation.
12. Giving orientation to newly joined pharmacists and pharmacy technicians. Delivering lectures for monthly report for intervention committees.
13. Reviewing and making new policies and procedure guidelines for pharmacy use.
14. Always welcoming new ideas to improve the work for better patient care.
