I evaluated their pharmacological therapies and determined if the medications, dosage and dosing interval are appropriate. I also provided medication therapy management (MTM) for patients with chronic disease states including but not limited to diabetes, hypertension, COPD, asthma, and long-term anticoagulant use. In this rotation, I was responsible for delivering medication therapy management (MTM) to improve patient outcomes and quality measures. I counseled patient on their pharmacological and dietary plan in diabetic clinic with collaborative practice. I became very familiar with variable diabetes therapy regimens and able to give counseling about injection techniques to patients under the supervision of my preceptor. I improved my clinical critical thinking by answering patients’ immunization and medication -related questions during immunization clinics and MTM sessions. I assessed patients’ pharmacological therapies, adherence’ barriers and drug-drug interactions by working up patients rounding notes and patient cases.
