Most Recent Accomplishments: Provide ongoing provider initiated program of cultural competent education in diabetes self-management to individuals and minorities in underserved communities. Successful implementation of this project is noted in the number of individuals that have taken charge of their health, reduced their risk of co-morbidities associated with diabetes and have shown decrease in re-hospitalizations as well.
