-Meet with patient and family and participate in Family care conferences to explain the role of palliative care and that I assess overall health, prognosis, expected disease trajectory, and quality of life, while helping caregivers and family members weight the benefits and burdens of treatment options.

-Present options for care while in hospital, including continuing will all aggressive cares and treatments, continuing current cares without escalation of care in condition declines or transition to more comfort focused cares.
