Home health primary care nurse practitioner for a small new company based out of Conroe, TX. While there, I observed a need for and implemented basic credentialing identifiers that were later adopted by management. The majority of patient population included geriatric chronically ill, multiple comorbity patients including many that were mobility impaired and/or disabled in some manner. Patients ranged from 28 years old up to 112 years old with the majority being older than 65 years old. Collaborated with Home Health nursing staff, Physical Therapy, Occupational Therapy and Speech Therapy staff in the treatment of our shared patients to coordinate care to reach the highest level of patient functional development.
