WORK EXPERIENCE

February 2015 – Present: MemorialCare Medical Group, Southern CA. Nurse Practitioner for leading multi-specialty health system located in LA and Orange Counties. I am the only Nurse Practitioner for an multi-physician Internal Medicine and Family Practice office located in Irvine, CA. Responsibilities include annual Complete Physical Exams,Well Woman Exams., Annual Wellness Exams, diagnosis and treatment of acute illnesses including respiratory, cardiac, musculoskeletal, sexually transmitted diseases, gastrointestinal, and urinary complaints. Diagnosis, treatment and management of chronic diseases including Hypertension, Diabetes Mellitus, Cholesterol, Thyroid disease, Contraception and Menopause management. Responsible for ordering and interpreting diagnostic tests, prescription management, referrals, coding, and Epic EMR. Selected to serve on Practice Transformation Committee.

July 2014 – February 2015: Alignment Healthcare, Orange, CA. Director of Nurse Practitioners and Physician Assistants. Worked hand in hand with Medical Directors and the Director of Clinical Operations to develop Medicare Advantage Program. Responsible for interviewing, hiring, supervising and training of all mid-level providers and Registered Nurses employed by Alignment Healthcare Care Centers and Home Programs throughout Southern California, Northern California, and North Carolina. Developed Standardized Procedure Manual for mid-level providers. Developed Chronic Disease Management programs including: Anti-Coagulation Clinic, COPD, CHF, Diabetes Mellitus, End Stage Renal Disease, Hypertension, Annual Medicare Wellness Exam, and Home Monitoring Program using Vivafy remote monitoring equipment. Participated in selection and training of AllScripts EMR.

May 2013 – July 2014: Irvine, CA. Nurse Practitioner for a private Internal Medicine practice. Common duties include: diagnosis and treatment for respiratory, urinary, musculoskeletal, gynecologic, and gastrointestinal complaints. Performed complete physical exams, well woman exams, school physicals, and pre-op physicals.

Responsible for ordering and interpreting diagnostic tests, prescription management, referrals, coding, and Practice Fusion EMR documentation.

August 2012 – March 2013: CareMore, Cerritos, CA. Nurse Practitioner. Responsible for the primary care needs of 70+ patients living in Assisted Living and Board and Care Facilities. Job functions included weekly and as needed visits to each patient involving H&P, assessment, formulation of treatment plan, and follow up of chronic and acute conditions. Duties included prescription management, referrals, coding, and NextGen EMR documentation.

Note: This was a six month temporary assignment.
